my 2 cents
post your schematics, grounding scheme, pics of the layout of the amp.
where is your speaker return coming to, should goto star ground. that causes problems. use twisted pair for power cables inside the amp, keeping them separate and away from signal cables.
is your dac on the same stabilizer/switch/mcb as the amp. try playing around with those.
