Lot of variation between manual & MCACC measurement, other than center & subwoofer.. Have them corrected..

Cross over settings in av receiver:
Front - large
Centre- large
Rest all speakers- small
Cross over frequency- 80 hz

You can have the fronts & center also set to small..

Should I select - main or main + sub or sub alone

If you are using with subwoofer (i.e. subwoofer=yes) then set the front left and right, center and surrounds speakers to small. This is irrespective of your drivers are large and capable of playing low frequencies, you should set them small and by doing it all crossover frequencies set below for these speakers (80hz in your case) will be redirected to your subwoofer and this will give you dynamic sound stage presentation.
 
Should I calibrate macc distance with manual measured distance

Never change the distance measurements calibrated by MCACC Eq. You could tweak the decibles + or - 2db but never change the distance measurements. In my opinion the calibration measurement does not work the way imagine it to be. It takes into account the time the sound waves takes to reach the mic to determine the settings.
 
Even though you have floor standing speakers it is always better to crossover to the subs. I have a full range speaker but I have crossed over at 40Hz. Try 40 or 60 Hz. For select sub alone. Audyssey mostly gets distances right but is too aggressive on the upper end which makes the sound felt sounding. Dirac and RoomPerfect are much better in that aspect. There is no single phase switch setting. It depends on room and placement. Please try different phase settings and see in which the bass is the strongest but not overbearing.
